  • ROCK’s year starts with summer camps in July followed by regular sessions in September through June.

 

Application Process for CURRENT participants:

  • Step 1: Participants will receive an email from ROCK that includes a link to the online application on the first Monday in May by 10 a.m. ( Please use the link in the email. Do not create a new application.)
  • Step 2: Click the link in the email and fill out the application.
  • Step 3: Once you electronically sign your application and submit, you will receive an email asking you to confirm your signature. Please be sure to follow the directions to confirm your signature. If you do not confirm your signature, your form will not be considered complete and will not be sent to ROCK.
  • Step 4: Download and print (Form 2 & 3) the Medical History/Physician Statement & Prescription form. Please return these forms, completed by your physician, no later than the first Monday in August. If we do not receive your medical release forms by then, you will not be put on the schedule until it is received. This could jeopardize the day and time you have requested. If you are receiving Physical or Speech therapy, you will also need the Physician Prescription Form filled out or your physician can write a script on his or her own prescription form.

Application Process for NEW participants: (Applications accepted starting the 1st Monday in June)

  • Step 2: Fill out participant application. Once you electronically sign your application and submit, you will receive an email asking you to confirm your signature. Please be sure to follow the directions to confirm your signature. If you do not confirm your signature, your form will not be considered complete and will not be sent to ROCK.

Summer Camp

  • If you have previously participated in Summer Camp, please contact Kristin at kristinw@rockride.org to receive the link to update your current application.
  • If you are NEW to Summer Camp, please follow the instructions above for the Application Process for NEW participants.
  • Download and print (Form 2) the Medical History/Physician Statement. Please return these forms to ROCK completed by your physician. Medical forms are due by the first Monday in July.
  • Summer camp applications are accepted the first Monday in April.

ROCK On Veterans

Application Process for NEW participants:

Application Process for CURRENT ROCK On Veteran participant

  • Please do not create a new application. Contact Heidi at Heidi@rockride.org to receive the link to update your current application.
